The first step to registration is to add your child to our wait list on https://regionofwaterloo.onehsn.com/

When a spot becomes available at the childcare centre, the Supervisor will contact you to offer you a spot. A registration checklist and all forms will be emailed to you to complete at that time.

Once you are offered a spot, and paperwork is complete, please book a registration appointment with the Supervisor by emailing childcare@macmaplegrove.com

• Financial Agreement
• Immunization
• Registration Checklist
• Childcare Parent Handbook
• Letter to parents regarding fee reduction pilot

In addition to the forms above, you will need:

• 2 copies of the immunization record
• 2 passport sized pictures of your child
• 2 copies of long form birth certificate
